There is the use of telephone calls to communicate with the ED lead nurse and the call to the anticoagulation clinic by the ED lead nurse to direct the patient at the clinic. The computerized order entry system is used by the nursing supervisor to enter the work necessary to dose Lovenox into the laboratory. Orders received from the provider for Lovenox and shipment of anti-coagulation are faxed to the clinic by the Lead ED RN. Policies and Rules There is a Lovenox Policy and Protocol in place within the organization. This policy must be followed when a patient needs to receive Lovenox injections; therefore, the policy is in effect from the beginning of the Lovenox flowchart. Lovenox policy determines the how, when and why of this flowchart. The who and where is left open for discussion and analysis. The anticoagulant clinic within the organization is open from 8.00 to 16.00, from Monday to Friday. Any patient diagnosed on an outpatient basis will follow the current workflow presented.
