Deals with the remuneration that must be paid for certain services. The funds for these services come from the premium paid by the insurance company. In most cases, insurance pays providers directly or, if under a managed care plan, patients are required to pay a copay and the rest is covered by the plan. The medical reimbursement system is created so that reimbursement occurs on a fee-for-service model. Essentially, the more services they order for their patients, the more money they make. The system financially rewards quantity of care over quality of care and therefore there is no incentive to encourage doctors to treat the entire continuum of care for their patients.
